Description

The dorsal column–medial lemniscus (DCML) pathway carries sensations of fine touch, vibration, conscious proprioception, pressure and two-point discrimination from the body to the cerebral cortex. First-order neurons ascend ipsilaterally through the fasciculus gracilis (lower limb) and fasciculus cuneatus (upper limb) before synapsing in the medulla. The fibers then decussate as internal arcuate fibers to form the medial lemniscus, ultimately reaching the ventral posterolateral (VPL) nucleus of the thalamus and the primary somatosensory cortex. Injury to this pathway results in loss of vibration sense, impaired position sense, sensory ataxia and a positive Romberg sign, affecting balance and coordinated movement.
